Ongoing Surveillance Testing
Beginning Monday, February 28, weekly testing is no longer required for individuals who have not provided proof of vaccination to the university. However, students, faculty and staff continue to have access to free COVID-19 PCR testing on campus. For more information on testing schedules and locations, please click here.
Tests can be PCR (molecular) tests or as an alternative two negative antigen tests (including home tests). Samples can be nasal or saliva. Check with the testing service you use to ensure the test is a molecular or PCR test.
Employees designated for surveillance testing will be notified by a supervisor.
You do not need to upload your results to the HealthyPack Portal if you have been tested at an on-campus testing site listed here.
To upload your results from any off-campus testing site, scan or take a photo of your documented results and save it to your device.

Documentation should include:
- Full Name or Initials
- Date of Birth
- Name of Laboratory or Testing Site
- Date of Test
- Type of Test
- Test results
- In the HealthyPack Portal, select COVID-19 from the Menu Options.
- Under “Enter External COVID-19 Lab Result” enter the date of your COVID-19 Test. Enter the result of the test. Select Submit.
- Above you will see the SELECT FILE button. Select this and attach your COVID-19 results documentation from your files. Select Save.
- Once the file has been selected, the file name will appear. Select Upload to attach the file.
- Your documentation will be reviewed by a Student Health staff member. If your documentation can not be verified, you will receive a secure message from student health with further instructions.
- If you are unable to access the HealthyPack Portal or having difficulty uploading your documentation, please call Student Health at 919.515.2563 for additional options and instructions.

You do not need to upload your results to the HealthyPack Portal if you have been tested at an on-campus testing site.
Yes, however in most cases your printed test results will suffice. If you were tested on-campus you can bring a printed copy of your on-campus COVID test results to the front desk in Student Health Services to have your test results stamped and/or certified as required for travel. We will not be able to alter the document by adding a sampling method or adding a QR code. Additional travel related COVID test questions can be sent to Healthpack@ncsu.edu.
You do not need an appointment at on-campus surveillance testing sites. If you are experiencing symptoms or have concerns about COVID-19 please contact Student Health at 919-515-2563 to schedule a telehealth appointment with a medical provider at Student Health Services. COVID testing will be available following your appointment, if appropriate.
The test that will be used is a nasal swab of the inside front of both the left and right nostrils. You will perform your own swabbing and place the cotton swab in a tube. Staff will be available to assist. View this short video on testing for a demonstration.
Students and staff are able to park in the SHS lot or in front of Witherspoon briefly while they are tested at the Witherspoon testing site. Additional parking has been designated at the other on-campus testing sites.
Effective, 5/17/21 parking passes will be required and enforced for the 12 reserved student appointment parking spaces in the SHS backlot.
- HealthyPack Portal is a medical portal for campus students and employees that will be used for uploading and reviewing COVID‑19 test results.
- HealthyPack Portal complies with HIPAA and FERPA information privacy requirements. The system uses the latest industry-standard security protocol and protects patient data through ongoing security checks and audits of the system.
- If we identify positive cases through any of the testing steps outlined above, NC State will conduct contact tracing and testing for all those potentially exposed, including students, faculty, and staff.
- Yes, for a limited time. Following a positive test result and isolation period, surveillance testing is not required for 90 days. If your test was from an off-campus testing location, please upload your test result into your HealthyPack Portal.
Testing locations are available across North Carolina. To find testing centers throughout the state, visit the NC DHHS Find My Testing Place resource page. Students, faculty and staff who choose to get tested out of state can check local listings for other locations.
Testing may also be available at local pharmacies, as well as urgent care centers or your doctor’s office. There may be a fee for testing or an office visit at these locations, and proof of insurance may be required.
If you are tested at an off-campus testing site as a part of the surveillance testing program you will need to upload your test results to the HealthyPack Portal.

COVID-19 Vaccinations
NC State is providing COVID-19 vaccinations to members of the Wolfpack community. If you have questions about how to get a vaccine on campus or your eligibility as a member of the NC State community and for any updates concerning vaccinations on campus please visit the Protect the Pack website.
If you have received a COVID-19 Vaccine (including boosters) at an off-campus location, you may upload it to the HealthyPack Portal so it becomes part of your medical record.
You will need to upload additional doses beyond a complete vaccine series (e.g. if you got a 3rd Pfizer dose, you will need to upload your vaccine record again).
Note: Any vaccine given at the PackVax Vaccination clinic on-campus is automatically added to the medical record and does not need to be uploaded to your HealthyPack Portal.

We are now offering COVID booster doses to eligible individuals. You do not have to receive the same vaccine type for your booster as received for your primary series (e.g. you may receive Pfizer as a booster if you initially received a Johnson and Johnson vaccine). If you do not meet the below criteria, you will not be offered a booster dose.
- We are now offering Pfizer as primary & booster doses to eligible individuals! If you do not meet the below criteria, you will not be offered additional doses at this time. You will only be offered the vaccine product you sign up for. See below for eligibility criteria.
- You are 12 years or older and it has been at least 5 months since you received your 2nd primary Pfizer or Moderna dose
- You are 18 years or older and it has been at least 5 months since your 2nd AstraZeneca (Covishield), Sinopharm, Sinovac, or Covaxin primary dose
- You are 18 years or older and it has been at least 2 months since your 1st Janssen (Johnson and Johnson) primary dose
CDC recommends anyone ages 12 years and older who are moderately or severely immunocompromised should receive an additional primary dose of COVID-19 vaccine. Visit the CDC's webpage for COVID vaccines for Immunocompromised individuals for more information on vaccine products and timing of doses.
Contact Tracing Program
- We are encouraging all NC State students, faculty and staff to confidentially self-report either a positive COVID-19 test results or a diagnosis of COVID-19 by a healthcare provider. This procedure is voluntary, and the information is confidential. Please fill out the relevant form to self-report:

- Collecting this information helps us understand and track the impact of COVID-19 on our campus, and allows us to follow up on any information that might affect the safety of faculty, staff, and students at NC State.
- If you were a close contact to a positive case you should:
- Follow strict mask wearing for the next 5 days
- Continue to monitor yourself for symptoms and get tested immediately if you develop cough, fever, sore throat, or unexplained weakness or tiredness
- Get tested in 3 - 5 days, whether you have symptoms or not. Please get tested on-campus if possible so results are received directly. If you get tested off-campus, upload your results immediately, especially if they are positive.

- Close contact: a close contact is defined as any individual who was within 6 feet of an infected person, with or without a mask, for at least 10-15 minutes starting from 2 days before illness onset (or, for asymptomatic patients, 2 days prior to positive specimen collection) until the time the patient is isolated.
- Physical Distancing/Social Distancing: this is maintaining a distance of 6 ft. or more between you and another person or group of people, in order to prevent the spread of viral infection in communities.
Most people who have COVID-19 begin to show symptoms two to 14 days after they’re exposed to the novel coronavirus. If you have any of the following symptoms, you should stay home and seek health care:
- Cough
- Shortness of breath or difficulty breathing
- Fever
- Chills
- Muscle pain
- New loss of taste or smell
- Sore throat (secondary symptom)
If you are experiencing any of the symptoms above and you are a student, please do not attend class, work or other campus activities and contact Student Health Services at 919-515-2563 for an appointment.
- Wear a mask.
- Getting vaccinated is the best way to prevent infection and help prevent the spread of COVID-19.
- Wash your hands with soap and water often for at least 20 seconds, and by using a hand sanitizer with at least 60% alcohol.
- Avoid touching your eyes, nose, and mouth.
- Practice physical distancing by staying at least 6 feet away from others.
- Clean and disinfect frequently touched surfaces daily.
- Stay home if you’re not feeling well.
If you have tested positive for COVID-19 and had the test done at an off-campus location, and are living on campus please call Student Health Services immediately. Students will be recommended to self-isolate due to illness for 5 days and, if asymptomatic at that time, you may leave isolation and continue to mask for 5 days to minimize the risk of infecting others. “Asymptomatic” means feeling well or you have no fever for 24 hours and improving cough and/or weakness. If you are in on-campus housing, you may do this at home with family or in campus provided isolation housing. If you test positive for COVID-19 off-campus, please self-isolate for at least 5 days and fill out the student self-report form.
SHS recommends following the CDC guidelines. For people who are unvaccinated or are more than six months out from their second mRNA dose (or more than 2 months after the J&J vaccine) and not yet boosted, CDC now recommends quarantine for 5 days followed by strict mask use for an additional 5 days. Alternatively, if a 5-day quarantine is not feasible, it is imperative that an exposed person wear a well-fitting mask at all times when around others for 10 days after exposure. Individuals who have received their booster shot do not need to quarantine following an exposure, but should wear a mask for 10 days after the exposure and consider getting tested 3 - 5 days after exposure.
Yes. Please fill out the Student Self-Report Form if you test positive for COVID-19 at a facility other than Student Health or an on-campus testing site. Students who are living either on or off-campus should let Campus Health know if they test positive.
Your health is our number one concern. We are taking extra measures to ensure the safety of our students and staff as we continue to provide care. We are enforcing social distancing measures through directional signage and increased seating spacing and reduced capacity in our waiting rooms. Face masks are also required while inside of Student Health. We have increased sanitation, improved building air ventilation, and adjusted our check-in procedure to reduce exposure and wait times within Student Health.
If you were in close contact (within 6 feet for 15 minutes or more) with an individual who has tested positive, you shoud:
- Follow strict mask wearing for the next 5 days
- Continue to monitor yourself for symptoms and get tested immediately if you develop cough, fever, sore throat, or unexplained weakness or tiredness
- Get tested in 3 - 5 days, whether you have symptoms or not. Please get tested on-campus if possible so results are received directly. If you get tested off-campus, upload your results immediately, especially if they are positive.

Unless you are a student enrolled in a Distance Education program, then you will pay regular tuition and fees and will have access to Student Health and other student fee supported services. Call us at 919.515.2563 to make an appointment.
Yes, all persons are required to wear a mask. If you have one, please wear it to your appointment. If you do not have a mask, please let our staff know when you are making your appointment and one will be provided to you when you arrive. Masks are also required while you are on campus and in all University buildings.
